Undergraduate students taking courses on the Macdonald Campus who are seeking consideration for tests/assignments/exams missed due to illness or other extenuating circumstances, must submit a request to the Macdonald Campus Student Affairs Office (SAO) using the form below.聽
You are never required to divulge specifics around your illness to your course instructor.
The SAO will review your request and any associated documentation (e.g. doctor's note) and email their decision to you and the relevant instructor(s) within two working days.聽After receiving the SAO's decision, it is your responsibility to discuss accommodation arrangements with your instructor.
The type of accommodation will be decided by the course instructor, in consultation with you. These accommodations may include, but are not limited to, handing in an assignment late, reweighting other assignments, or a make-up test.
Please note that if you are seeking accommodation in a course offered by another Faculty, you will need to follow their process.
